Causes of Prurigo:
Are considered to be more promising agents than real causative factors.
1 External factors: insect bites. Migratory parasites, allergic contact, physical factors such as heat, cold and light.
2 internal factors.
- Neurological tensions and several psychological factors of origin.
- thyroid gland factors.
- Sense of food and medicine.
Infections: An internal infection foci such as chronic tonsillitis and chronic sinusitis.
Differential diagnosis:
- Pelvic dermatitis.
- Neurodermatitis.
Chronic dermatitis.
Herpes.
Treatment:
- Topical treatments of low value.
- Avoid insecticides and insect bites.
Avoid excessive heat or cold skin.
- Intravenous injections: Steroidal steroids such as triaminolone are often useful.
- Antihistamines (Zyrt ec 10) mg or (Atara x 25) mg and 100 mg may help relieve itching.
- Sedatives: may be very useful especially in patients emotionally disturbed. Higher doses are given before bedtime in the evening, as itching is more severe in some patients because of the warmth of the skin especially in winter.
- Prolonged corticosteroids (Kenalog 40 mg or Depot Medrol, I.M): can be given every 24 weeks.
Benoxaprofen: Also used successfully in some cases.
Thalidomide: It is probably the most effective treatment if it is not contraindicated because of a dangerous pregnancy, as there is some painful neuropathy.
